Patient Assessment App is a comprehensive healthcare automation solution designed to modernize patient assessments, clinical documentation, and healthcare workflow management. Built using Microsoft Power Platform, Azure AI services, and SharePoint technologies, the platform enables healthcare providers to efficiently manage patient records, conduct interviews, generate AI-assisted assessments, and automate medical documentation. The solution begins with a patient onboarding process where client information is submitted through Microsoft Forms and automatically stored in SharePoint. Upon submission, a dedicated SharePoint site is provisioned for each patient, creating a centralized repository for patient records, assessments, documents, and generated medical forms. Healthcare professionals can access patient records through a custom Power Apps interface that provides patient information, appointment tracking, interview history, clinical notes, and AI-assisted assessment tools. During consultations, doctors can record conversations directly within the application. Audio recordings are securely stored and transcribed using Azure AI services, enabling automatic generation of consultation summaries, suggested next steps, and potential diagnoses. The platform leverages Azure OpenAI to analyze patient conversations and identify possible treatment recommendations, follow-up actions, and required medical procedures. When specific medical services such as X-rays, laboratory tests, or physical examinations are mentioned, the system automatically retrieves predefined templates, populates patient information, and generates documents for physician review. Additional capabilities include appointment management, interview history tracking, AI-powered clinical summaries, patient-specific document storage, automated notifications, and intelligent workflow automation. The solution significantly reduces administrative workload while improving the quality and consistency of clinical documentation.
